The Center For Family Health Care is owned and operated by
two board-certified nurse practitioners. We firmly believe
that our patients are our most valuable asset, And we are
committed to treating you with care and integrity, knowing
that this will, in turn, develop an exemplary health care
practice and will be evident in the quality of our health
care services.
Our Patient Centered
Model Of Care
The Center For Family Health Care is a patient centered primary
care practice. This approach encourages adherence to medical
advice and improves a patient's health status. "Patient centeredness"
has been identified of the key components of quality health
care.
Our services at The Center For Family Health Care are modeled
after the seven attributes of patient centeredness. Those
attributes are:
- Superb access to care. Patients can easily
make appointments and select the day and time. Waiting times
are short. E-mail and telephone consultations are offered.
Off-hours service will be available.
- Patient engagement in care. Patients have
the option of being informed and engaged partners in their
care. Our practice will provide information on treatment
plans, preventive and follow-up care reminders, access to
medical records, assistance with self-care, and counseling.
- Clinical software information systems that support
high-quality care, practice-based learning, and quality
improvement. Our practice will monitor adherence
to treatment; have easy access to lab and test results;
and receive reminders, decision support, and information
on recommended treatments.
- Care coordination. Specialist care is coordinated,
and systems are in place to prevent errors that occur when
multiple physicians are involved. Post referral follow-up
and support is provided.
- Integrated and comprehensive team care.
There is a free flow of communication among the nurse practitioners,
physician and other health professionals. Duplication of
tests and procedures is avoided.
- Routine patient feedback to providers.
Our practice will take advantage of patient surveys to learn
from patients what needs to be done to improve our service.
- Publicly available information. Patients
have accurate, standardized information about our practice
that will assist them in meeting their health care needs
via our website and My Health Portfolio.
